For placement into treatment, please have a recent substance use placement screening. If you have not completed a substance use placement screening with a recommendation for one our levels of care, then you can schedule an appointment to do so CADS outpatient location. They will then assist you with next steps to our location.

If you have already completed a substance use placement screening with a recommendation to one of our levels of care, you can have the referral emailed to us at CADS_CoReferrals@unitypoint.org or faxed to (563) 333-9108.  Additional information may be requested to ensure appropriateness and success in placement at our facility.
