Country Oaks Location

Some additional information to be aware of is as follows. If you have any concerns with the below expectations, please give us a call so we can answer any questions or help problem solve any barriers/concerns.

● Clients should arrive to treatment appropriately detoxed from substances and not under the influence, as we do not have detox onsite. We understand this can be challenging and for safety reasons may require support at a detox or medical facility. Feel free to give us a call so we can help provide you guidance and support with this challenge.

● If arriving in your own vehicle, please make sure you bring proof of a current and valid driver’s license, vehicle registration, and vehicle insurance. Vehicles will not be allowed to stay on property without this verification. If you are leaving your vehicle onsite, we suggest that you park towards the back of the lot, particularly to avoid needing to move your vehicle for plowing in inclement weather.

● Clients should have supply (ideally 30 days but we understand refills may not be ready yet) of any medications and corresponding medical supplies. Medications should be in original bottles, current, non-expired, and appropriately labeled. Releases of information may be requested to confirm any concerns or questions from prescribing providers and/or pharmacies. Please note that medication approvals at our facility are at the discretion of our approving provider.

● If you have paperwork that is needing to be filled out for purposes of such as disability, FMLA, return to work, etc., please connect with your primary care provider pertaining to completion of these forms, as this is who would be monitoring your care long-term.

● The facility is a tobacco free property to all visitors and staff. Clients are strongly encouraged and supported with tobacco cessation efforts.  However, clients may use tobacco in a small designated area outside only.

● Individuals are strongly encouraged not to bring valuables into the facility.  Items such as cell phones are locked up and available for use during designated or approved times.  While basic bedding, bath, hygiene, and laundry supplies are available, clients may also bring their own in.  A seven day supply of clothing, basic necessities, and small amount of general items for during free time (eg. notebook, reading book, coloring book, personal mp3 player with headphones, etc.) is encouraged. 

●Sealed bottle water is allowed, but outside food or drink for Residential, Day Treatment, and Outpatient with Housing is limited to medical/supervisor approval based off of individual dietary, religious, and/or treatment needs.  If this is a need, please chat with us prior to admission.

OWI-2 (Outpatient with Housing)

For those that are fulfilling legal requirement of 7 days of treatment at Country Oaks in lieu of 7 day incarceration time, please be aware of the following.

● Upfront payment is typically needed for those completing outpatient with housing option for room & board costs of the 7 day requirement.

● While someone may complete the 7 day legal requirement, to successfully complete the treatment program, one also needs to complete and fulfill all subsequent clinical recommendations as well.
